Our Strategic Plan for 2026 to 2027 outlines how Belong at Home will continue to deliver high quality, person centred aged care and in home support services across the South West of Western Australia.
As a trusted local not for profit provider, our focus is supporting older people to live independently, safely and connected in their own homes, now and into the future.
Our Direction
This plan sets a clear direction for the next two years, guided by our commitment to quality care, strong communities and continuous improvement. Through our Umbrella Framework, we ensure every client receives care that is personalised, safe, respectful and connected.
We are committed to delivering services that respect each person’s individuality, support their wellbeing and help them stay connected to what matters most.
Our Priorities
Over this period, we will focus on:
- Delivering flexible, responsive home care and in home support services.
- Supporting older Australians to remain living at home with confidence and independence.
- Building a skilled, supported and values led workforce.
- Strengthening governance, quality and compliance systems.
- Enhancing technology and service delivery to improve client outcomes.
- Preparing for aged care reforms, including the Support at Home program.
- Growing strong partnerships across the South West community.
